Image Consent Forms

Dear All,

You may remember some time ago you were asked to complete a consent form giving Richmond permission to use photographs that you are featured in on our website and social media platforms.

The forms were completed in April 2019 in person at ‘The House’. You then were asked this time last year if you were happy for the forms to roll over for the year 2020.

We now kindly ask that you complete and submit a new consent from via the link below for the year 2021.

Please complete the form, even if you don’t wish for photos of you to be used.

All results will be kept strictly confidential and your decision will be respected.

We ask that parents sign and complete a form for each of their children (under 18) if they attend, Teens Group, Sunday School or Tots Group.
